Objectives

Fellows will

  1. Demonstrate ambulatory care that is cost-effective and presents best practice.

  2. Recognize the presentation and management of common cancers, including their epidemiology, evaluation, prognosis, treatment, patterns of advanced or metastatic disease, emergencies, complications, associated symptoms, and symptomatic treatments.

  3. Develop symptom management skills with a particular focus on prevention of unnecessary hospital admissions.

  4. Collaborate with all elements of the palliative care continuum including primary care physicians, referring clinicians, hospitals, palliative care units, nursing homes, inpatient hospice, insurance companies, pharmacies, and other community resources.

  5. Participate in tetradirectional teaching and learning; attending, fellow, pharmD are all learning from each other, the patient, and family.

Please be on-time and ready to see patients.

Please page the attending if you are running late.

The clinic staff pages the attending when the patient arrives and the attending may begin the visit if the fellow is late.

Location

Meet in the fellows room. Patients are seen on Yawkey 11 or other floors as needed

Preparation

Review chart prior to appointment if possible including LMR notes, medications, radiology reports, labs, and appointment schedule to know who else is involved in the patient's care.

Notes

Dictated or typed into LMR. See note template.

Fellows are encouraged to try both means of documentation.

Notes must be completed prior to leaving clinic for the night. Save in prelim for the attending to review and sign.

Typed into LMR within 24 hrs. Use LMR templates for new consults and follow-up notes. Save in prelim for the attending to review and sign.

Fellowship requirements

Fellows will have clinic for six months at either the DFCI or MGH, and then swap sides

If you miss a session, there are opportunities to make up sessions on other days.
